Please carefully read through the following terms and conditions for the 6-pack proposal: 

I commit to showing up on time, prepared, having completed my agenda & ready to participate to the best of my ability for my coaching calls. 

I understand I am responsible for taking notes during the session and while Amy will likely jot down notes for us both it's my responsibility to either request permission to record the session and/or take notes. 

I understand it is my responsibility to schedule my coaching calls via the calendar link supplied in the welcome email. 

I understand I am responsible for paying the full price of the contract amount of $1860.00 whether or not I personally choose to complete all three sessions. 

I agree to cancel any appointment (s) I can't attend by canceling via the scheduling link or by directly emailing Amy 24 hours prior to our scheduled call. 

I commit to creating an agenda for all three calls & loading in our shared Google Drive Folder PRIOR or emailing it to [email protected] AT LEAST 24 hours prior to our scheduled call. 

I acknowledge that Amy will guide me through this reimbursement process BUT I am fully responsible for executing the tasks requested of me. 

I understand email support isn't included in the consulting package. I agree I will address all my questions & concerns ON the call not via email.

I understand if I select a payment plan, and default more >7 days on my payments, services including any remaining calls, information shared in Google Drive & email access will be suspended. 

I acknowledge Amy's services are not consistent with those of a credentialing agency or billing agency. She will teach me how to execute these tasks but will not do them for me.
